Interesting referral to that book. I'm genuinely curious and now want to read it. I know two people who died despite extended CPR so this is interesting to me.
Regarding the advance health directive: this is why my brother has not been designated my health care advocate. I do trust him to follow my wishes, but I don't trust what my being in extremis might do to him. He was a little surprised by this decision but he respected it, which I respect him for doing. He's awesome that way.
And yes, I hope neither of us will have to test any of this work any time in the near future.
